What is Doxycycline (for Malaria Prevention)?
Doxycycline is a broad-spectrum tetracycline antibiotic that has been in medical use since the 1960s. Alongside its role in treating bacterial infections, Doxycycline is widely used as an antimalarial prophylactic (preventive medicine) for travellers visiting regions with Chloroquine-resistant malaria.
For Spanish Vibracina, the prophylaxis indication concerns falciparum malaria in areas with mefloquine resistance, or where other antimalarials cannot be used in chloroquine-resistant areas. This is not an indication to prevent traveller’s diarrhoea.
How does Doxycycline work?
Doxycycline inhibits protein synthesis in the parasite. UKMEAG classifies it as suppressive prophylaxis directed at blood-stage parasites, rather than predominantly liver-stage prevention. Continue daily prophylaxis for 4 weeks after leaving the risk area to cover parasites emerging into the blood. It does not eliminate dormant liver hypnozoites.
Who is Doxycycline for?
A doctor may consider doxycycline for eligible adult travellers. The Spanish Vibracina label requires particular justification for children aged 8 to under 12 when alternatives are unavailable, ineffective or contraindicated; paediatric dosing is product- and weight-specific. It is particularly considered for:
- Travellers to sub-Saharan Africa, Southeast Asia, and Oceania
- Travellers who cannot tolerate Malarone or Mefloquine
- Longer-term travellers where product-dependent treatment cost is a consideration
Do not use doxycycline for this prophylaxis in:
- Pregnant women (risk to developing foetus)
- Breastfeeding women
- Children under 8 years of age (risk of permanent tooth discolouration and bone effects)
- People with known hypersensitivity to tetracyclines
Dosage
The Spanish Vibracina adult malaria-prevention schedule is:
| Phase | Adult Dose | Timing |
|---|---|---|
| Pre-travel | 100 mg once daily | Start 1–2 days before departure |
| During travel | 100 mg once daily | Continue throughout stay |
| Post-travel | 100 mg once daily | Continue for 4 weeks after leaving malaria area |
Doxycycline should be taken at the same time each day, with a full glass of water and ideally with food or milk to reduce gastric side effects. For Vibracina, allow at least 1 hour before lying down or going to bed to reduce oesophageal injury.
Side effects
Possible side effects
- Gastrointestinal effects: Nausea, vomiting, and abdominal discomfort — most common and often reduced by taking with food
- Photosensitivity: Increased susceptibility to sunburn. Use SPF 30+ sunscreen and protective clothing
- Oesophageal irritation: Risk increases when taken without sufficient fluid or immediately before lying down
- Vaginal candidiasis (thrush): Can occur after disruption of normal vaginal flora
Other reported effects
- Diarrhoea
- Headache
- Skin rash
- Tooth discolouration (in children under 8 — a key contraindication)
Serious reactions requiring assessment
- Intracranial hypertension (pseudotumour cerebri): Rare; presents with severe headache and visual changes. Stop treatment and seek medical attention immediately.
- Severe skin reactions: Including erythema multiforme and Stevens-Johnson syndrome (rare)
- Hepatotoxicity: Liver toxicity, particularly with prolonged high-dose use
Doxycycline vs other antimalarials
| Medicine | Dosing | Destination considerations | Cost consideration | Post-travel duration |
|---|---|---|---|---|
| Doxycycline | Daily | Check itinerary and contraindications | Varies by product | 4 weeks |
| Malarone (Atovaquone/Proguanil) | Daily | Check itinerary and contraindications | Varies by product | 7 days |
| Lariam (Mefloquine) | Weekly | Avoid resistant areas | Varies by product | 4 weeks |
| Chloroquine | Weekly | Sensitive areas only | Varies by product | 4 weeks |
Compared with Malarone, doxycycline requires a longer post-travel course and careful sun protection. Actual costs and access depend on the product and lawful supply route.

Important safety information
Do not take Doxycycline if you:
- Are pregnant or planning to become pregnant
- Are breastfeeding
- Are under 8 years of age
- Have a known hypersensitivity to doxycycline, tetracyclines or excipients
- Are receiving methoxyflurane (risk of fatal kidney toxicity)
Tell your doctor if you:
- Have liver disease (dose adjustment may be needed)
- Have oesophageal problems or difficulty swallowing
- Are taking oral contraceptives (Spanish label and UK guidance differ; obtain product-specific advice)
- Are taking antacids, iron supplements, or calcium supplements (reduce Doxycycline absorption — take separately)
- Are taking warfarin or other anticoagulants (Doxycycline may enhance anticoagulant effect)
- Have myasthenia gravis (tetracyclines may worsen muscle weakness)
Drug interactions: Separate antacids and supplements containing calcium, magnesium, aluminium or iron as instructed by the prescriber or pharmacist; they can reduce absorption. Do not treat milk as equivalent to these products: Vibracina permits administration with food or milk. Avoid concomitant isotretinoin because of intracranial hypertension risk. Prolonged treatment requires periodic blood counts and liver/kidney monitoring. Severe or persistent diarrhoea during or after treatment needs assessment for C. difficile.

Frequently asked questions
- How does Doxycycline prevent malaria?
- Doxycycline inhibits protein synthesis in malaria parasites and acts as suppressive, blood-stage prophylaxis. It can be an option where parasites resist chloroquine or mefloquine, after an itinerary-specific assessment.
- How long do I need to take Doxycycline after returning from a malaria area?
- Take the prescribed prophylaxis daily for 4 weeks after leaving the malaria-risk area. This covers parasites reaching the blood after exposure; do not shorten the course on the assumption that doxycycline mainly acts in the liver.
- Does Doxycycline cause sun sensitivity?
- Yes. Doxycycline commonly causes photosensitivity — increased sensitivity of the skin to ultraviolet (UV) light. Travellers taking Doxycycline should use high-factor sunscreen (SPF 30 or above), protective clothing, and avoid prolonged sun exposure, particularly during peak UV hours.
- Can women take Doxycycline for malaria prevention?
- Doxycycline must not be used for malaria prevention during pregnancy or breastfeeding. Vibracina warns about oral contraceptive interactions, whereas UKMEAG says additional precautions are not routinely needed with this non-enzyme-inducing antibiotic unless vomiting or diarrhoea occurs. Ask the prescriber or pharmacist to resolve advice for your contraceptive and product; do not disregard the Spanish warning.
- Can I get Doxycycline for malaria online in Europe?
- An independent doctor can assess your trip and health history. Prescription and lawful supply are not guaranteed; an online assessment is not evidence of legal delivery in Spain.
