What is Aldara?

Aldara (imiquimod 5% cream) is a prescription topical medicine used to treat external genital and perianal warts caused by certain strains of the human papillomavirus (HPV). It is approved by the European Medicines Agency (EMA) and classified as an immune response modifier rather than a direct antiviral agent.

Unlike treatments that physically destroy wart tissue, Aldara works by engaging the body’s own immune system to target HPV-infected cells. This mechanism distinguishes it from other wart treatments such as podophyllotoxin (Condyline, Warticon) and cryotherapy.

How does Aldara work?

Imiquimod, the active ingredient in Aldara, acts as a toll-like receptor 7 (TLR7) agonist. When applied to wart tissue, it binds to immune cells in the skin and triggers a cascade of immune responses:

• Interferon production: Imiquimod stimulates the release of interferon-alpha and other cytokines, which activate the immune system’s antiviral response at the site of application.
• Cytokine cascade: The immune activation leads to local production of tumour necrosis factor-alpha (TNF-alpha) and interleukins, which promote immune cell infiltration into wart tissue.
• Cell-mediated immunity: T-lymphocytes are recruited to the treatment area, where they target and eliminate HPV-infected keratinocytes.
• Indirect antiviral effect: Because imiquimod acts on immune cells rather than directly on the virus, it does not kill HPV; instead, it helps the immune system clear infected tissue.

This immune-mediated mechanism explains why local skin reactions (redness, erosion, flaking) are both expected and indicative that the medicine is working.

Who is Aldara for?

Aldara is indicated for adults with external genital and perianal warts. It is not suitable for:

• Internal warts: Aldara is for external use only and should not be applied inside the vagina, urethra, or rectum.
• Children and adolescents: The safety and efficacy of imiquimod in patients under 18 years have not been established for this indication.
• Pregnant women: There is limited safety data for imiquimod in pregnancy. A doctor must carefully assess the risk-benefit balance.
• Patients with compromised immune systems: Imiquimod’s mechanism depends on an intact immune response; effectiveness may be reduced in significantly immunocompromised individuals.

Posologia e applicazione

Aldara 5% cream is applied three times per week (for example, Monday, Wednesday, Friday) before normal sleeping hours, typically for a period of 6 to 16 weeks.

Istruzioni per l’applicazione (as per EMA-approved guidance):

1. Wash the treatment area with mild soap and water and allow to dry.
2. Apply a thin layer of Aldara cream to the warts and rub in gently until absorbed.
3. Leave the cream on for 6 to 10 hours, then wash off with soap and water.
4. Do not cover the treated area with an occlusive dressing.
5. Avoid sexual contact while the cream is on the skin.

Interazioni farmacologiche: Imiquimod is applied topically and systemic drug interactions are not well documented; however, avoid use with other topical products on the same area simultaneously unless directed by a doctor.

Aldara does not treat the underlying HPV infection and does not prevent transmission to sexual partners. Barrier contraception (condoms) reduces but does not eliminate the risk of HPV transmission.
