What is Canesten Gyno?

Canesten Gyno (clotrimazole) is an antifungal medicine used to treat vaginal candidiasis, commonly referred to as vaginal thrush or a yeast infection. It is one of the most widely used antifungal treatments in Europe and is listed on the WHO Model List of Essential Medicines.

Canesten Gyno is available as vaginal pessaries (tablets inserted into the vagina) in doses of 100 mg, 200 mg, and 500 mg, as well as external cream for symptomatic relief of vulval itching and irritation. The 500 mg single-dose pessary is the most commonly prescribed formulation for convenience and adherence.

How does Canesten Gyno work?

Clotrimazole belongs to the azole class of antifungals. It works by inhibiting an enzyme called lanosterol 14-alpha-demethylase, which is essential for the synthesis of ergosterol — a critical component of the fungal cell membrane. Without adequate ergosterol:

• The fungal cell membrane becomes structurally unstable
• Cell membrane permeability increases, allowing intracellular contents to leak out
• Fungal cell growth and replication are halted (fungistatic effect)
• At higher concentrations, clotrimazole can also directly kill fungal cells (fungicidal effect)

This mechanism selectively targets fungal cells while leaving human cells largely unaffected, because human cell membranes use cholesterol rather than ergosterol as their primary sterol component.

Who is Canesten Gyno for?

Canesten Gyno is indicated for adult women with vaginal candidiasis caused by Candida species (most commonly Candida albicans). It is appropriate for women who:

• Have confirmed or strongly suspected vaginal thrush based on symptoms (itching, white cottage-cheese-like discharge, vulval soreness)
• Have previously had a confirmed diagnosis of vaginal candidiasis and recognise the same symptoms

Medical assessment is recommended before self-treating if:

• This is your first suspected episode of vaginal thrush
• You are pregnant or breastfeeding
• You are under 16 years of age
• You have had more than two infections in the past six months
• You have unusual symptoms (sores, abnormal discharge, pelvic pain, fever)
• You have diabetes, HIV, or are immunocompromised

Dosage

The appropriate Canesten Gyno formulation depends on the severity and recurrence of the infection, as specified by the prescribing doctor.

Application: Pessaries should be inserted as high as possible into the vagina using the applicator provided, preferably at bedtime. The cream is applied externally to the vulva and surrounding skin to relieve itching.

Side effects

Canesten Gyno is generally well tolerated. Local reactions are the most commonly reported effects.

Common side effects

• Vaginal burning or stinging (particularly immediately after insertion)
• Vaginal itching (may temporarily worsen before improving)
• Vaginal discharge (may increase initially)
• Mild pelvic discomfort or cramping

Less common side effects

• Skin rash or redness at the application site
• Vulval swelling or oedema
• Peeling or blistering of skin (with external cream)

Rare but serious effects

• Systemic allergic reactions are extremely rare with vaginal application due to minimal systemic absorption

• Canesten Gyno vs Nystan: Nystatin requires a longer treatment course (14 days) and is generally less convenient. It is typically reserved for patients where azoles are not appropriate.
